- Implementation Science
- CHIIS implementation science training provides instruction in the skill sets needed to deliver the triple aim of better care and better health at lower cost, focusing on the theoretical and applied knowledge to successfully localize, implement, and evaluate evidence-based practices, as well as innovate and invent new models of care and processes.
- Population Health Management
- Our Population Health Management course will help individuals understand the need for change, the differences between the traditional case management model and the population health management model, and implement and deliver the changes required to manage the care of a population/panel of patients across the continuum of care.
